GDPR Consent Forms

16 May 2018

Dear Member,

As you may be aware, new EU regulations regarding the protection of personal data are coming into force at the end of May this year.

This regulation is called “General Data Protection Regulation” (GDPR) and part of GDPR requires that any organisation that collects, stores, processes and shares personal data about EU citizens must have permission to do so and that permission must come from the person to whom the data applies.

It is also a requirement that each person should be aware of what specific data is being stored and how it is processed.

To effectively administer the membership of Carrickmines Croquet and Lawn Tennis Club (CCLTC) we need to collect, store, process and share certain personal data about each of our members.

The Consent Forms (listed below) detail the personal data and its purpose.  We have also introduced a new Data Protection Policy for the club which is available to collect from the Club office or by email to

The Club GDPR Policy will also be posted in the members section of the club website in due course. This policy details how we protect your personal data.

In order to enable your membership of CCLTC to be administered properly and to ensure CCLTC complies with the new regulations please complete the attached Consent Form and return to my attention at your earliest convenience.  Consent Forms are also available to download from the club website and can be collected from the Office or Bar.  If you have any queries please let me know and I will endeavour to address them for you.

I look forward to receiving your signed form shortly.

Best Regards,

John Doyle,

GDPR Officer
